Has she had a miscarriage or is she pregnant?

To get whats happend i need to explain from the start... My gf accidently missed her Birth control pill. So to stop her having an almost endless period (it lasted nearly 3 months) she came off the pill for a bit and we restrained ourselves... Later she then went back on the pill, but it goes wrong here we had before the recommended two week wait and i didnt withdraw. When we had again, she gushed quite a bit of blood which we later noticed to have alot of tissue like substance in it. It had no resemblance to a normal period it looked very light in colour and not to mention i know when shes on. There was no breakthrough or further bleeding afterwards, it was just one gush and it was over...? But now shes feeling a bit of pain during and still slightly bloated with cramps included. Also i noticed her cervix was in a much different place than usual, much closer to the l opening and softer than usual. So any ideas would help thanks.
